Core service
Skilled nursing, in the comfort of home

A familiar setting. A clinical helping hand.
Coming home after a hospital stay can bring questions about new medications, dressings and what to watch for. Nursing visits bring focused clinical support into your home, while helping you and your family understand the next steps.
Skilled nursing is offered in Texas and Illinois (Cook, Lake and DuPage counties). Contact us to confirm availability in your area.
Recovery support
Follow-up care after hospitalization or surgery, guided by your physician's orders and individual care plan.
Medication confidence
Reviewing medications, explaining instructions and helping you recognize questions to discuss with your prescriber.
Changes noticed
Monitoring symptoms and vital signs, documenting concerns and communicating relevant changes to your care team.

How it works
- Call us or have your doctor send a referral.
- A registered nurse completes an in-home assessment.
- We build a written plan of care with your physician and family.
- Nurses visit on schedule and report changes to your doctor.
Skilled services require a physician's order. Services are provided under our Texas HCSSA license.
What a nursing visit may look like
Your nurse reviews how you have been feeling, checks the concerns identified in your care plan and provides the ordered treatment. There is time to discuss medications, symptoms and practical questions about recovery. Relevant findings are documented and communicated to your physician.
Helping families feel prepared
Teaching is part of care: how to follow care instructions, use prescribed equipment and recognize signs that need medical attention. With your permission, a family member or representative can take part in these conversations.
Planning your first conversation
Have your location, physician's contact details, discharge instructions and a current medication list available if possible. Our coordinator confirms whether we can meet your needs, what orders are required and the private-pay cost before care starts. Use a secure method agreed with our team to share medical records.
Home nursing is not an emergency service. For severe or sudden symptoms, call 911.
